All students MUST register, including those who may wish to apply for a transfer. Speak to an administrator about Transfers.
If you are NOT ACCEPTING a placement at the school, parents MUST complete a Parent Declaration Form. Speak with our school administrator for further assistance.
Parents/Students are advised to have ALL relevant documents before filling out the registration form.
Complete ONE of the registration options below by Wednesday 05th July, 2023.
Form 1 Registration form (downloaded or collected at school)
Student passport size photo
Original and One (1) Copy of
Secondary Entrance Assessment (SEA) placement slip
Student Birth Certificate - must carry the same name as the placement slip
An Affidavit is required when there is no name on the birth certificate, or if the name on the birth certificate does not correspond with that on the placement slip.
Valid Student's Permit/Evidence of Immigration Status (applies to Non-Nationals)
Family data: Parent/Guardian National ID, Driver's Permit or Passport of at least ONE parent/guardian.
Collect the registration form at the school on or before the registration date.
Return with the completed registration form and ALL required documents on registration day.
Fill out form online, download and print OR Download and fill out form in BLOCK LETTERS using a black or blue ink pen.
Return with the completed registration form and ALL required documents on registration day.
Note: Type NA if a response is required but does not apply to you!
